Mindanao Educators Mutual Benefit Association, Inc. is a non-stock association duly registered with Securities and Exchange Commission on August 4, 2011. It caters primarily to all educators in Mindanao
In order to meet our goals and objectives, MEMBA maintains a wide range of qualifications. We have a set of highly skilled team that provides supreme customer service.
The exceptional services and well-experienced team coupled with the intensive implementation of our objectives makes Mindanao Educators Mutual Benefit Association, Inc. the primary mutual benefit association for educators.
MEMBA provides its members with financial assistance and death benefit, through successful and consistent implementation of our programs.

POLICIES

1. Completely filled-out Application Form with no erasures.
2. Net Take Home Pay (NTHP) of Php 6,500.00 and above.
3. Latest Original Payslip and Photocopy of the two (2) preceding months.
4. Original and Photocopy of Updated Service Record & Appointment paper.
5. Two (2) 1X1 ID pictures (latest & not cut-out).
6. Php180.00 for initial MSC and premium.
7. Present original and photocopy of two (2) valid ID’s – PRC, GSIS or DepEd ID.
8. Not more that 55 years of age.

3. Should be a member in good standing.
4. Completely filled-out application form with no erasures.
5. Net Take Home Pay (NTHP) of 6,500.00 with no undeducted obligation.
6. Latest Original Payslip and Photocopy of the two (2) preceding months.
7. Copy of Verifier’s confirmation for financial assistance approval.

1. Maintaining balance of member’s supplemental contribution is P1.500.00.
2. The FAR balance should be less than the total members’ supplemental contribution in which case the difference can be withdrawn.
3. Withdrawals cannot be made simultaneously with financial assistance application.

1. MEMBA Passbook, ID and Membership Certificate.
2. Original Death Certificate from the Local Civil Registrar.
3. If beneficiary is:

Surviving Spouse: Photocopy of Marriage Contract (bring original LCR) and photocopy of two (2) valid government ID.

Mother/Father: Photocopy of Birth Certificate of deceased member (bring original LCR) and photocopy of two (2) valid government ID.

Child/Brother/Sister: Photocopy of Birth Certificate of beneficiary (bring original LCR) and photocopy. Minor: Affidavit of support of surviving Spouse/ or guardian and photocopy of two (2) valid government ID

4. Principal certification of the last day of service.
5. Certification of last Payment from PSU.
1. Upon the death of a member covered under Mutual Benefit Plan, his/her beneficiaries shall be entitled to Mutual Benefit Coverage of P50,000.00 provided that he/she was a member in good standing. * A member is said to be in good standing, when he/she has no unpaid obligations.
2. All unpaid obligations of the member to MEMBA such as membership fees and contributions, loan payments, unpaid fees and contributions shall be deducted from the benefits due.
3. Claims for the mutual benefit shall be filed by the beneficiaries of the deceased member within six (6) months from the date of death appearing on the death certificate.
4. Only in cases of the death of the designated Primary beneficiary will the mutual benefits of the member be given to the secondary beneficiary. If primary beneficiary cannot claim for whatever reason, he/she must execute an Special Power of Attorney, in favor of the secondary beneficiary.
5. In case, both beneficiaries are deceased and policy was not updated by the member, the mutual benefit can ONLY be claimed by the immediate family.
1. Surrender MEMBA Passbook, ID & Membership Certificate.
2. Updated Service Record.
3. Photocopy of latest payslip with no MEMBA deduction.
4. If retired;
Certification of Last payment from DepEd Payroll Services Unit. If resigned/ transferred; Principal certification stating the last day of service.
1. Final settlement of your account will be completed upon the stoppage of all deductions
2. All deposits to member’s supplemental contribution can only be withdrawn if all deductions are settled.
